  1. Student Name
  2. Student Age and Year in School
  3. Name of School
  4. Town or School District
  5. A brief description of the way in which the nominee has worked to promote sleep health and/or safe, healthy school hours (e.g., forming a sleep or later school start time club, writing an editorial about sleep health and/or school start times, working with school leaders to change school policy, etc.)
  6. A link to a website or article confirming the work described in #5 OR a statement from a school or community leader or teacher confirming the work.
All nominations are sent with the understood permission by the student, the student's parents or guardians (if the student is under 18 years old), and the nominator to publish any of the information included in the nomination - including the student's name, school year, town, and work - on Start School Later's public website and other social media (e.g., Facebook and Twitter).
